To submit an access request via our online form, please read the following choices carefully and choose the option that best describes your situation:

Request to access information options

You are an individual wanting to access Personal information regarding yourself, OR you are an individual acting on behalf of someone else to access their personal information (must provide proof of authorization ie. Guardianship, Power of Attorney) OR you are requesting the police report about the sudden death of a family member.

You are an individual wanting access to General Information (ie: Statistics, Calgary Police Service policies, Crime-related incidents that occurred at public locations, etc.).

You are a Landlord wanting to access information regarding a property that you own.

You are an employee of an Insurance Company and wanting to access information to process a claim.

You are a business OR a law firm acting on behalf of a business in the capacity of their legal counsel (including business owner, property management company, etc.) wanting to access incidents related to your business.

You are a law firm acting on behalf of a client in the capacity as their legal representative and wanting to access their Personal Information.

All ID verification photos are used to verify your identity regarding records you have requested and will be deleted once processing of your request has been completed. We do not retain ID photos for future requests.

ID / Selfie exclusion

We are unable to process your online request without the upload of your ID/Selfie, however, you can print off a PDF copy of the form (download PDF) and attend a District Office or CPS headquarters along with a copy of your government issued photo ID for verification in person by an Officer.

Accepted forms of authorization include: Signed authorization/consent form, Guardianship, Power of Attorney, Will documentation and Executorship
